Admissions
At St. Monica Academy, we strive to keep the admission and registration process simple. It begins when you complete the registration form, tour the school and meet with the principal or attend an open house. Each family will be provided with a registration folder that contains a registration form and additional helpful information.
In addition to the registration form & tuition agreement, families should be prepared to provide the following documents:
- Official birth certificate or passport.
- Baptismal Certificate (if the child is Catholic and intends to participate in preparation for Holy Communion, Reconciliation, and/or Confirmation)
- Immunization Record and all required exams as per the State of Illinois. Most pediatricians will have these documents for you. If not, forms are available in the Forms and Documents Library.
- Transfer students will also be asked to provide a copy of the most recent report card and other pertinent educational documents (Eg: Individual Education Plan (IEP) or 504 Plan)

Important note: Grade placement is based on the child's age as of September 1st of the school year. Example: Only if the child is at least 3 years old on September 1st can s/he be placed in PreK3.
